Can anyone give me tips on how to upgrade equipment?

For example, what's the safest upgrade(+) number?

Every level of refine is safe.

I see. And the max level would be 10,correct?

Everyone says only +10 the weapons due to fear of thana. However, I disagree to that. If you already have 2xskolls and ring, chances are your def is already negative, which makes non-incant weapons(2xTG 2x skelworker for example) hurt you more. Having +10 armors can save you from that and even if you had +10 on everything, the skolls and rings makes your def low enough to not give enough damage to thana. But then again it's up to you.
